DEFINED CONTRIBUTION PENSION FORMS

Defined Contribution Pension Forms:

Distribution for Benefits Form
To apply for a withdrawal, you will need to complete an Application for Benefits. Please complete the form and mail along with any noted documentation to the Fund Office.
Incoming Rollover Form
To roll funds into the plan from another qualified retirement plan or IRA complete the form and include a copy of a check stub listing the rollover contributions and/or earnings amounts or a current account statement. The check from your former plan must be made out as shown on the bottom of this form. Please return the form to the Fund Office.
Certification of Disability Form
To apply for a withdrawal from your account due to disability complete the form and return with the application for benefits. Please note that if you were a member of the Drywall Plan, and are requesting a distribution of your Account M balance, you must also submit a copy of your SSA award letter. Please return both the application for benefits and your Certification of Disability forms to the Fund Office.
Application for Hardship Distribution (Account P) Form
To apply for a hardship withdrawal due to medical expenses or to prevent eviction or foreclosure of the participant’s primary residence from account (P), complete this form and return it to the Fund Office. Please note you must also include the proper documentation to support the hardship for which you are applying. The type of documentation required by the plan is listed on the application.
Application for Hardship Distribution (Account M) Form
To apply for a hardship withdrawal due to eviction or foreclosure of the participant’s primary residence from account (M), complete this form and return it to the Fund Office. Please note you must also include a copy of an eviction or foreclosure notice. The notice should identify the total amount past due, which months are past due, and any additional fees incurred.
Investment Election Form
To change your investment allocation you will need to contact Milliman, the Fund Recordkeeper, either by phone at (866) 767-1212 or online.
Change of Address Form
Complete this form to change or correct your mailing address, and return it to the Fund Office.
Change of Name Form
Complete this form to change or correct your name, and return it to the Fund Office.
Beneficiary Designation Form
To designate a beneficiary for your account, you must fill out this form and return it to the Fund Office.

Summary of DC Accounts:

Participants in the Plan may have up to three (3) sub-accounts within his/her Individual Account. The sub-accounts are known as Account M, Account P, and Account R.

* Account M applies to those participants who previously participated in either the Minneapolis Local 386 Drywall Finishing Industry Pension Fund or the Minnesota Glaziers and Allied Trades Retirement Fund prior to the merger of those retirement plans.
* Account P refers to contributions made to the Painters & Allied Trades DC82 Defined Contribution Pension Plan (generally contributions made on and after October 1, 2016).
* Account R refers any rollovers received by the Plan.
